Walking in for the first time

Trying a new pharmacy can feel like a hassle. You wonder what paperwork you need, whether your prescriptions will transfer, and how long it will take. At Healthcare Providers Inc. in Hamilton, the first visit is simple, and it is also a good chance to get a clear picture of everything you take.

What to bring

  • Your insurance card, plus any prescription discount or supplemental cards

  • Your medicines or a list of them, including the name, strength and how you take each one

  • Vitamins, supplements and over the counter products you use regularly

  • A list for anyone you care for, like a parent or neighbor, if you help manage their medicines

  • Your questions, written down so you do not forget them at the counter

If you are moving prescriptions from another pharmacy, the bottles are especially helpful because they show the prescription number and the pharmacy they came from.

What a medication review is

A medication review is a sit down conversation with a pharmacist about everything you take. Some people call it a brown bag review, because the easiest way to do it is to put every bottle, box and tube in a bag and bring it in.

We go through it together, one item at a time. It is not a test, and there are no wrong answers. The goal is simply to make sure everything you take makes sense together.

What a review often turns up

You may be surprised what is in the cabinet. Common finds include:

  • Duplicates. The same medicine under a brand name and a generic name, or two products with the same active ingredient, like two pain relievers that both contain acetaminophen.

  • Interactions. A prescription that does not mix well with an over the counter product or supplement.

  • Old or expired medicines. Bottles from years ago that are no longer prescribed or no longer good.

  • Leftover prescriptions that were stopped but never thrown away, which can cause confusion later.

  • Timing problems. Medicines that work better taken at a different time of day, with food, or apart from each other.

  • Cold and allergy products that are not a good fit for someone with high blood pressure or other conditions.

Anything that needs a change in a prescription goes back to your doctor. Our job is to spot it and explain it clearly.

Who benefits most

Anyone can ask for a review, but it is especially helpful if you:

  • Take several medicines every day

  • See more than one doctor or specialist

  • Recently came home from the hospital or a procedure

  • Just started a new prescription

  • Have trouble keeping track of doses

  • Help a parent, spouse or neighbor manage their medicines

For caregivers in the Bitterroot Valley, a review can take a lot of the guesswork out of managing someone else's prescriptions, especially when they come from different doctors.

After the review

You leave with a clearer list of what you take and why. If something should be discussed with your doctor, we will tell you what to ask. If old medicines need to go, we can talk through safe ways to dispose of them.

It is a good idea to repeat a review once a year, or any time a new medicine is added or a hospital stay changes your list.
